Breast-Preserving Surgery for Breast Cancer

Abstract
Currently, it is well known that breast-preserving surgery and post-operative radiation therapy is as effective as modified radical mastectomy for breast cancer. We reviewed 3 patients who undrwent breast-preserving surgery for primary breast
cancer at
Kangnam St. Mary's Hospital Catholic University Medical College during the period of 4 years from January 1989 to December 1992.
@ES The results are as follows:
@ES 1) The prevalent rate of breast preserving surgery is 10.8% (23 out of 212 cases)
2) Age specific percentage of the preserving surgery shows peak in their 30's.
3) The most common initial manifestation is palpable mass and the most frequent site of the lesion is upper outer quardrant(56.5%).
4) The most common pathologic type is invasive ductal cancer(86.4%).
5) According to TNM classification, there are 17 cases(74.0%) is Stage I, followed by 5 cases (21.7%) in Stage II and I case(4.3%) in Stage III.
6) Diagnostic accuracy by mammogram and ultrasonogram is 59.1%(13 out of 22 cases) and 77.8%(7 out of 9 cases) respectively.
7) Hormone receptor status in evaluable 12 cases revealed that ER(+), PgR(+): 2 cases. ER(+), PgR(-): 3 cases, ER(-), PgR(+): 0 case, ER(-) PgR(-): 7cases.
8) All patients received post-operative radiation therapy and mild dysconfiguration is found in all cases but most of patient are satisfied with cosmetic outcome. The pigmentation and dryness of skin are the most common sequalae (87.0%)
following
fibrosis of the treated breast, radiation pneumonitis and limited motion of shoulder in decreasing order of frequence. postoperative asymmetry of nipple is problem to be solved.
9) Average follow up period is 24.9 months and 2 cases were recurred in 7 and 26 months after surgery(recurrence rate: 8.7%, disease free interval: 22.7 months). One case was treated by salvage mastectomy but another case refuse operation.
During
the
follow up, no death case In spite of short follow-up periods, the author may conclude that breast-preserving surgery provide acceptable results, compared to modified radical mastectomy, in terms of diease free survival
